Most of the time you can request more medication without a follow-up. A few situations need a check-in with your provider first — here's how to tell which one you're in.

You've Had A Consult In The Last Six Months
New patient or follow-up, either counts. A follow-up is required every six months so your provider can monitor your progress.

It's The Same Medication As Last Time
Switching between medication types — semaglutide to tirzepatide, or the reverse — needs a follow-up consultation.

You're Staying At, Below, Or One Level Above Your Last Dose
You can refill at the same dose, a lower one, or one level up — not further. This applies to every plan type.

It Isn't Your First Refill Of A GLP Medication
Starting a GLP-1 for the first time, you get at least two months of treatment before a follow-up is required. On a monthly plan that's one refill after your initial prescription, with the follow-up due before the order after that. On a tier or extended plan your first order already covers that stretch, so the follow-up comes before your first refill.
Already on a GLP-1 before joining us? If you provided documentation of your prior treatment, you can typically refill without a follow-up.
Changing pharmacy is always fine. You can switch pharmacies on a refill without needing a consult — check the Pharmacy State Coverage list first to confirm your new choice ships to your state.
Same Dose, Lower, Or One Step Up
This applies to every plan type — monthly, tier, extended, and brand-name alike. Compared to your last order, you can request:

Two Or More Levels Up
Doses step up one level at a time. Increases are deliberately gradual so your body has time to adjust and side effects stay manageable. A provider can make an exception in rare cases, but it's a clinical judgment based on your history — not something a follow-up automatically unlocks.
Two things change this. If you're in the maintenance program, you'll continue at your current dose rather than stepping up. And if your provider has a reason they don't want you increasing right now, they'll tell you directly — so if you haven't heard otherwise, the rule above is what applies to you.
When A Follow-Up Is Required
Any one of these means completing a follow-up rather than submitting a refill request. The same waiting periods apply — see the table above for your plan.

You Just Started A GLP For The First Time
A check-in after the first round lets your provider see how you're tolerating it before continuing. If you came to us already established on a GLP-1 and provided documentation, this usually doesn't apply to you.

Your Last Consult Was Over Six Months Ago
You need a consult at least every six months to keep refilling. If your provider has placed you in the maintenance program, that stretches to every twelve months.

You Want To Switch Medication Types
Moving between semaglutide and tirzepatide is a clinical decision, not a refill change.
The same goes if you want to move from an injectable GLP to an oral one. Orals work differently, so your provider will walk you through how the option you're considering works and what to expect from it before you make the switch.

You'd Like To Be Designated As Maintenance
Maintenance status is requested during a follow-up. It moves you to once-a-year check-ins instead of every six months. Whether you qualify is at your provider's discretion, based on your treatment history and how stable you are.
Beyond-Use Dates And Why Tier Choice Matters
Your Medication Has A Use-By Date
The Beyond-Use Date, or BUD, is the last day your compounded medication should be used. It runs from 60 to 90 days, measured from the date the medication was made — not the date it arrived.
That's why tier choice matters. Ordering a larger supply than your dose will get through means part of it may expire before you reach it.
Pick the tier that matches your prescribed dose, so you finish the supply inside the BUD. The Treatment Plan & Pricing page lays out which tier suits which dose.
You can request a refill as soon as your waiting period is up, but plan around the BUD rather than the earliest allowed date — that's how you avoid both gaps in treatment and medication you can't use.
