Once you're stable and thriving on your treatment, you may qualify for our maintenance program — a simpler rhythm of care designed for long-term success.

At Lavender Sky Health, we want care to feel as simple as it is effective. The maintenance program recognizes patients who have reached a steady, successful place in their treatment — and gives them the room to keep going with less paperwork and fewer follow-ups.

This page explains what maintenance is, who qualifies, and how to talk with your provider about it.

What does "maintenance" mean?

Being categorized as a maintenance patient means you are no longer actively adjusting your medication dose and have reached a stable treatment plan that's working well for you.

Typically, maintenance patients:

  • Have met or are maintaining their treatment goals
  • Are stable on their current medication and dose
  • Are tolerating treatment well without needing frequent adjustments

How does maintenance affect my care?

Maintenance simplifies your follow-up rhythm without changing what's working.

Patients categorized as maintenance:

  • Only need a provider follow-up once every 12 months instead of every 6 months
  • May continue refilling at the same dose, tier, or plan while on maintenance
  • May also refill an equivalent plan option when appropriate (for example, Tier 2 or a Momentum Plan equivalent)
If your treatment needs change in the future, your provider can always reassess and adjust your plan.

Who is a good candidate for maintenance?

Maintenance is individualized and determined by your provider based on your medical history and treatment response.

Patients who may be good candidates often:

  • Have achieved their treatment goals or reached a stable weight
  • Have remained on a consistent dose for a period of time
  • Are looking for a longer-term treatment approach without frequent dose changes

How do I request maintenance status?

To be considered for maintenance, request it during a follow-up consultation.

Your provider will review:

  • Your dose and titration history
  • Weight trends and treatment progress
  • Medication tolerance and side effects
  • Overall treatment stability and safety
The goal is to determine whether it's appropriate and safe for you to transition to less frequent follow-up visits.

Why might I not be approved for maintenance yet?

Not every patient is ready for maintenance at the same time — and not being approved does not mean you cannot stay on your current treatment plan.

Your provider may recommend more frequent follow-up if there are factors that need closer monitoring, such as:

  • Ongoing dose adjustments
  • Recent side effects or tolerability concerns
  • Significant recent weight changes
  • Certain medical or clinical history considerations
For example, patients with a lower-normal BMI may still qualify for maintenance, but some patients may benefit from closer follow-up depending on their individual history and needs. These decisions are made with your safety and long-term success in mind.

Can I stay on the same dose even if I'm not approved for maintenance?

Maintenance status only determines how frequently you need provider follow-up visits. Even if you are not categorized as maintenance, you may still remain on the same medication, dose, or plan if your provider feels it's appropriate for your treatment.

What if I need to change my dose later?

That is completely okay. If you are categorized as maintenance and later need to:

  • Increase your dose
  • Decrease your dose
  • Change plans or treatment approach

You'll simply complete a follow-up consultation so your provider can reassess your care and make any needed adjustments.

At that point, you'd return to active treatment status and resume standard follow-up timing (typically every 6 months) until maintenance becomes appropriate again.
