Medical Records
Your chart — consultation notes, treatment history, and documentation of your care.
Prescription Record
Proof that a medication was prescribed to you, and by whom.
Itemized Receipt
A detailed breakdown of what you paid for. This is the document an HSA or FSA administrator asks for, and what you'd submit if you're seeking reimbursement.
Travel Documentation
A letter confirming your prescription for airport security. If you're flying with injectable medication, needles, or cold packs, request this before you travel.
For anything other than full medical records, email support and say which document you need. Put the document type in your subject line — Prescription Record, Itemized Receipt, or Travel Documentation — so it routes to the right person immediately.

Records are processed and sent typically within 5 business days, sometimes longer depending on our queue.
Need it sooner? If you're requesting prescription proof, or documentation for insurance or your HSA, say so when you ask — we'll prioritize it where we can.
Before You Request
Plan Ahead For Travel
Five business days is a week of calendar time once a weekend lands in the middle. If you're flying, request your travel letter as soon as you book, not the week you leave.
Requests Come From You
We can only release records to the patient on the account, or to someone you've authorized in writing. If another clinic is requesting them on your behalf, they'll need your signed authorization.
