Follow-up consults are required every 6 months for patients on active treatment and every 9 months for patients on maintenance dosing. Follow-up consultations include medication refill.

Become a Patient
To get started with us as a new patient, you will complete either a New Patient E-Consult OR a New Patient Phone Consult:
New Patient E-Consult 🗨️
New Patient E-Consults (electronic consults) are completed asynchronously via secure portal messages with a provider (physician or nurse practitioner).
You will complete a New Patient E-Consult Form and within 3 business days, a provider will review your form and send you a consult message via the patient portal. You may reply to their message directly to discuss your treatment plan and ask any questions you may have.
New Patient E-Consults do not require (or include) a phone call or appointment for ultimate convenience.
New Patient Phone Consult 📞
New Patient Phone Consults are a live call with a provider (physician or nurse practitioner) to form and discuss a treatment plan for those who prefer to talk to their provider in real time.
You will complete a New Patient Phone Consult Form to access a link to book with a provider (subject to scheduling availability).
Medication Assisted Weight Management & Wellness Injections
GLP-1 Agonists 💉
GLP-1 agonists are highly sought after for weight loss treatment. They have been shown to help patients on average lose up to 15-20% of their overall body weight. If you and your healthcare provider select a GLP-1 agonist as part of your treatment plan, you have the option for us to send your prescription to your local pharmacy for pickup or we can send your prescription to a specialty pharmacy for fulfillment. The accredited specialty pharmacies we work with offer customizable formulations and dosages to meet your needs. When you order through a specialty pharmacy, the medication is shipped directly to your door.

We currently do not provide prior authorizations for any medications. Please check with your insurance for their requirements if you plan to use it for purchase at your local pharmacy.
Brand name medication prescriptions are subject to your local pharmacy's availability and are in high demand. Please check with your pharmacy before we send your prescription that they have availability for your medication/dose.
If you choose a specialty pharmacy to fulfill your prescription, please note that pricing, availability, and fulfillment times are at the discretion of the pharmacy. You can view current pricing for specialty pharmacy medications here.
If you choose to order from a specialty pharmacy, you are able to choose from any of our five established pharmacies as long as they are licensed in your state. You may view specialty pharmacy state coverage here.
B Vitamin Boosters 💉
When combined with a healthy diet and exercise, lipotropic B-vitamin injections have assisted patients in achieving healthier body composition, as well as helping to regulate sleep, increase mood & mental clarity, and boost the immune system. In-Home Wellness Labs
We now offer in home labs that provide testing of 17 critical biomarkers to measure inflammation, hormones, metabolic, and cardiovascular health. These biomarkers are an excellent way to get a comprehensive look at your general health and wellness. For weight loss patients, this testing helps us best advise you on how to be successful, and allows another measurement of your achievements beyond just the scale.
The test kit is shipped directly to your home. Once you open the kit, you will find clear and detailed instructions on how to use the kit as well as customer support if needed. Only 8 drops of blood are needed which you can self-collect. After your test kit is shipped back to the lab, you will have results in 3-5 days. You will have access to an advanced dashboard with personalized recommendations based on your results.
Once you have your lab results, you can review with your provider at your next follow-up, or you may request a provider consultation for a detailed review of your results anytime.
