Important Note Regarding Lily Direct:
We submit prescriptions directly to Lilly Direct via fax only. They will then reach out to you for payment and shipping information.

Before submitting a refill request or follow-up, please review the following:

Requesting Standard/Brand Name Medications via Refill Request
Consultation Requirement:
You must have had a consultation within the last six months (New Patient Consult or Follow-Up).
If your provider has placed you on maintenance, you must’ve had a consult in the last nine months.
Timing:
You can request a refill 21 days after your last GLP invoice payment date or RX Call-In Request.
Medication Type:
You may only submit a refill request if you meet the criteria above and are requesting the same medication type.
(Example: Tirzepatide → Zepbound).

Requesting Standard/Brand Name Medications via Follow-Up
Consultation Requirement:
If you have not completed a consultation within the last six months, you must schedule a Follow-Up appointment.
Medication Type:
A Follow-Up appointment with your provider is required if you are switching medication types
(Example: Semaglutide → Zepbound).
If you meet the follow-up criteria above and would like to proceed, please submit a follow-up form.
